General Information on the Criminal Background Check
- Accepted Dental Hygiene or Dental Assistant Program students must complete the criminal background check process and email the "Criminal Background Check Statement of Understanding" to the Dental Department Director at [email protected] by the specified deadline.
- Students must request a criminal background check from the specified vendor, pay the fees, and authorize the Dental Department to access the results by the deadline. The vendor will compare the applicant's record against a list of reviewable crimes (permanent, ten-year, and five-year). If there is a match, the applicant may be notified of potential disqualification. Disqualified individuals can challenge their record's accuracy with the vendor, using the contact information provided in the Program's letter.
- Disqualified students who believe they no longer pose a threat can request an "Appeal for Reconsideration of Exclusion" by scheduling an appointment with the program director by calling 541-956-7370.
- Certain felony convictions within the past seven years, specific drug convictions, multiple DUIs, serious misdemeanors, or a computer crime may make a student ineligible for appeal. For DUI convictions within 18 months of the Program's start, an appeal requires proof of completing a diversion or treatment program at least 12 months before the Program begins. Students with pending legal issues won't be considered for an appeal until their case is resolved.
