Instructions for the CWE Agreement Packet

  1. CWE Agreement and Learning Objectives are received by CWE office.
  2. Student registration is complete

After student and instructor have arranged a CWE training site, all pages of this packet must be filled out completely.

The CWE Application Packet is now processed through DocuSign electronically. Faculty should be prepared with the following information before beginning the application:

  1. Student Name and Email
  2. Student Major/Program
  3. CWE Course/Section#
  4. Credit/Clock hours approved
  5. Employer Name and Email
  6. Is Student Paid or Unpaid (note: unpaid CWE requires RCC to provide workers’ compensation)
  7. Student’s Job Title
  8. Learning Objectives and Evaluation Criteria

Student, instructor, and employer must agree to all pertinent items within this Agreement Packet, signing where indicated in the document. CWE instructors, employers and students should retain copies of all CWE Agreement Packet pages for the term. Instructors should retain any assessment materials, including timesheets, evaluation forms, and grading records for one year.

Once completed and signed by faculty, the application will go to the student and the employer to complete their portions of the application and sign. It will then be reviewed by the CWE coordinator and routed to the CWE office to register the student. A final copy will be emailed to all parties once the student is registered.

  • CWE students placed at any RCC Campus to complete CWE hours must also complete a Volunteer Agreement Form. Fill out form and submit with volunteer (student) and RCC supervisor signatures. The CWE office will obtain the "Department Head/VP/Dean" and "Human Resources" signature(s) required. If your student will be doing any of the following:
  • Have unsupervised contact with students/clients
  • Unsupervised access to handle money
  • Work with data sensitive information
  • Operate dangerous equipment

You will need to complete a criminal background check (CBC).

In addition:

  • Some CWE sites (such as Asante, Providence, and many school districts), may have additional pre-placement processes. This might include immunization verification, criminal background checks, or other requirements. CWE credits cannot be registered until these processes are complete. The CWE office will need confirmation of “clearance” issued by the appropriate HR office.
  • Student and CWE instructor will be notified via email when CWE credits are registered. Credits are usually registered within one business day. If there is missing information or further questions, student and instructor will also be alerted right away.
  • Please do not use permission codes (at any time) to register CWE credits. The CWE office or (after the add/drop deadline) Rogue Central Services should be responsible for registering CWE credits.
  • CWE credits added after the add/drop deadline have an additional process. After paperwork is received by the CWE office, student must complete registration (in-person) with Rogue Central Services (RCS). RCS will complete registration and arrange payment of credits for student.
  • Students must communicate any changes with their CWE site to their CWE instructor. CWE credits should not be dropped or withdrawn without discussing the situation with their CWE instructor. If a student moves from an "unpaid" to "paid" status, that must be communicated to CWE office. It is important the unpaid hours worked are reported for Workers' Compensation purposes.
  • Please make sure student is aware of the RCC Student Code of Conduct and Procedures, found through the RCC Website.
