Application Instructions
Before proceeding to the application below, read this document. Failure to follow the instructions may result in your application being rejected. Our department does not accept any hand-written or hand-submitted applications.
- Review the application and its requirements carefully before completing and remember the following:
- You must meet with an Emergency Services Advisor BEFORE completing this application. If you do not have an Emergency Services advisor, click here to get in touch!
- Make sure to review the application checklist FIRST
- Have a copy of all relevant certificates and a copy of your Oregon driver’s License available as you will need to upload them while completing the application.
- If there are errors and it is before the deadline, Nadean will reach out with an opportunity to re-submit a new completed application.
