Amount:
One applicant will be awarded a $1000 scholarship for first prize, one applicant will be awarded $500 for runner-up, and three applicants will be awarded $250 for honorable mention.
Description:
The Law Office of Cohen & Jaffe, LLP Scholarship Program is designed to help high school students (upon graduation from high school) or college students who are interested in attending an institution of higher education. We value education as a vital foundation for creating fulfilling, advantageous careers. We are proud and excited to help students achieve their goals of furthering their education.

REQUIREMENTS:

The following documents must be provided for consideration (.doc, .docx, or .pdf format only) to the attention of Richard Jaffe via email only to [email protected].

1) An official copy of a current academic transcript.
2) Proof of legal residency in U.S. (i.e. birth certificate, passport, driver’s license, permanent resident card, etc.).
3) Proof of acceptance to an institution of higher learning. Should a winner be chosen that is still waiting on an acceptance letter, candidate will have until May 15th to provide proof of acceptance before forfeiting the prize.
4) A one-page essay of no less than 500 and no more than 750 words on one of the following topics:

Whether it be from a critical error made during a procedure, or not paying attention, there are a number of reasons why patients are harmed or injured at medical facilities every day in the U.S. At the Law Office of Cohen & Jaffe, we fight for justice when someone is harmed. Considering your research, what guidelines would you develop to ensure safety precautions are adhered to at medical facilities? What needs to be changed to protect patients? Tell us your ideas.

2) Be accepted to an institution of higher learning.
3) Be graduating high school, receiving GED within 9 months or college students.
4) Scholarship program available to undergraduates only.
5) Employees of the Law Office of Cohen & Jaffe, LLP, Consultwebs.com, Inc., their boards of directors and immediate family members are not eligible for this essay contest.
