•The award amount is $2500 over 2 semesters. •Applicants must have been 15 or older during active treatment or •Applicants must have been 15 or older when their parent/sibling was in active treatment. •Winner is obligated to 40 hours of community service.

Step1: Please read the Frequently Asked Questions (FAQs) document (found on the website). The information in this document will help you choose the correct application and successfully complete the application form- remember, if you qualify for multiple UCF scholarships, you only need fill out ONE application. We recommend you save/print this document as reference while you are filling out your application.
Step 2: Click on the application you wish to submit (on the website). Note: the applications are Microsoft Word documents with expandable fields.
Step 3: Save the application you have chosen on your computer.
Step 4: Complete the application (per the instructions), changing and saving your file as you would any Microsoft Word document.
Step 5: When you have finished, print the application and sign and date it as indicated.
Step 6: Mail the application and all required attachments (per the instructions). The mailing address is shown above.
Note: Many scholarships have similar criteria- only ONE application is required per applicant. The application will be evaluated for every scholarship for which the student qualifies. The only excecption is the Sean Silver Memorial Scholarship for a student in active treatment. That application is different and must be filled out separately.
