Academic & Behavior Information Release: I want others to have access to my student record.

Measles Immunization Status: I was born after 1957 and am enrolled in dental assisting, nursing, nursing assistant, practical nursing, EMS, human services, or early childhood education.
Military Credit Request: I want credit for classes/training I took in the service.
Official Transcript Request: I want my RCC transcript sent to another school.; I want another school to send a transcript to RCC.; I want my GED Transcript/Test Records.
Oregon National Guard/Selected Reserves Tuition Award: I was honorable discharged and want to go to school.
Parking Permit: I need an RCC parking permit.
Petition to Change Academic Record: I think there has been a mistake in calculating my grade.
Principal's Approval: I am under 18 years of age and want to attend RCC.
Repeat Course: I want to replace a lower grade with a more recent grade.
Student Account Petition: A hardship prevented me from finishing a class.
